Report a Lost or Stolen Passport

If your passport has been lost, stolen or damaged, you must appear in person at the U.S. Consulate to obtain a new passport. If you have emergency travel, It is best to call in advance if you need an emergency passport. Please call the Consular Section at: (01) 313 39 7535. Otherwise, please schedule an appointment before coming.

If you are replacing a lost or stolen passport you need:

  • Form DS-11 (pdf/101kb), Application for a U.S. Passport (disregard the instructions on the application as they apply in the United States).
  • Form DS-64 (pdf/51kb), Statement Regarding a Lost or Stolen Passport.
  • Police report documenting incident of theft.
  • Proof of U.S. citizenship (i.e. U.S. birth certificate, Naturalization Certificate, Consular Report of Birth Abroad, Certificate of Citizenship, previously issued U.S. passport).
  • Photo Identification.
  • One photo, see photo instructions.
  • A self-addressed stamped return envelope.
    TO PICK-UP THE PASSPORT IN PERSON IS NOT AN OPTION!  Except for a limited emergency passport which will be issued within one working day!
    Attention: For all passport applications you must submit a self-addressed and pre-paid envelope for return registered mail!
    For maximum security, we will only return passports by registered mail, please follow as instructed;
    Please provide stamps in the amount of € 6.50. Get a “Einschreiben/Registered” sticker from your local post office. Fill in the sticker (do not attach it to the envelope) and attach only the stamps on your self-addressed envelope (preferably size A5).

  • All applicants issued a Social Security Number, including children under 16, must provide it on their passport application forms in accordance with 22 U.S.C. 2714a and 22 C.F.R. 51.60(f)

If you are replacing a damaged passport you need:

  •  Form DS-11 (pdf/101/kb), Application for a U.S. Passport (disregard the instructions on the application as they apply in the United States)
  • Your damaged U.S. passport.

You will be asked to fill out a sworn statement detailing the nature of the damage and the circumstances which led to the damage when you speak with a Consular Officer.
